Nutrition recommendations can differ for different aspects of fertility & reproductive health. If you'd like more targeted information in your report, please provide some more information on any health conditions/ other relevant information:

Please list any nutrition supplements that you take and your dose E.g., Omega 3s Nordic Naturals 650mg x2/day

Food Diary:

Please provide me with a detailed 24 hour food diary. Include absolutely everything that you eat in a day and where possible, specific brands and amounts. E.g. breakfast: 1/2 cup plain rolled oats + 1/2 cup greek easiyo plain yoghurt + 1 banana + 2 Tbsp LSA. etc.

Make sure to include all snacks and drinks including those that may not happen every day- just to give me a good idea of your usual intake. E.g. after dinner 1 row chocolate + red wine every Friday/ Saturday 2 glasses (example)

If you can provide me with more than 24 hours then this is even better!
